Richter’s Syndrome: A Case Report

Richter’s syndrome is the development of a malignant Iymphoma in a patient with preexisting chronic lymphocytic leukemia (CLL). This well-documented phenomenon is generally considered to represent a monoclonal proliferation of B-lymphocytes arising from CLL. In this case we describe a patient in whom CLL and non-Hodgkin lymphoma were diagnosed simultaneously. A heterogeneous population of cells consisting of sheets of transformed lymphocytes in combination with clusters of bizarre, large pleomorphic cells with abundant pyroninophilic cytoplasm and large irregular nuclei with prominent nucleoli which sometimes closely resemble immunoblasts. Immunophenotypic studies in the peripheral blood suggest that morphologically abnormal lymphoblastic cells most likely represent an abnormal clone originating from B-lymphoid lineage.
